If you are an adopted person searching for your birth parent(s), you can contact the Ohio Department of Health, Vital Statistics Unit. Visit them on-line at www.odh.state.oh.us or write them at P.O. Box 15098, Columbus, Ohio, 43215-0098. Birsth Certificates are available for adoptions before 1969. After that date, the probate court in the county in which the adoption took place may release certain non-identifying information at their discretion. This information also applies to birth parents and relatives seeking a child that was placed for adoption.
